This visit helps you keep your health on the right track. Don’t have a doctor? Consider a patient-centered medical home, which may offer extended hours, classes, and visits with a nurse and pharmacist.
To find a patient-centered medical home near you, call the Medicare Concierge Team at (401) 277-2958 or 1-800-267-0439 (TTY: 711). Our hours are Monday through Friday, 8:00 a.m. to 8:00 p.m., Saturday and Sunday, from 8:00 a.m. to noon (open seven days a week, 8:00 a.m. to 8:00 p.m., from October 1 - March 31). You can use our automated answering system outside of these hours.
If diabetes, heart disease, and other health problems are not under control, they can lead to complications and even hospital stays. Talk to your doctor about what you can do to best manage your health. You may be able to work with a nurse at your patient-centered medical home.

These screenings can find diseases in earlier, more treatable stages and, in some cases, even help prevent them. Depending on your age and gender, this could include screenings for colon cancer and breast cancer. Ask your doctor what screenings you should receive.
Regular exercise lowers your risk for heart disease, stroke, some cancers, diabetes, high blood pressure, depression, and much more. It also improves sleep and strengthens your bones.
